Upload the plugin ZIP from the downloads page in WordPress → Plugins → Add New, and activate it.

Paste your CareHQ Account ID and API key into the CareHQ tab. The plugin verifies the connection and pulls in your care homes.

Map your Gravity Forms form

Open the Mappings tab, pick your Gravity Forms form, drag CareHQ fields onto your form fields, and save. Submissions start syncing immediately.

Yes. The connector uses the official Gravity Forms hooks API and is tested against the current and previous major versions.

Can I send the same Gravity Forms entry to two different care homes?

Yes — conditional routing lets you send each entry to one CareHQ location based on a form field. Sending the same entry to multiple homes is on the roadmap.
