Yes. The plugin connects to CareHQ using API credentials generated in your CareHQ account.

What are the requirements?

- An active CareHQ account with API access
Is the connection secure?
Yes. All data is sent over HTTPS with API key authentication.
Can I use it with multiple forms?
Yes. Each form can be configured independently with its own field mappings, routing rules, and default location.
What happens if a submission fails?
The form still completes normally for the user. Failed submissions are logged in the Logs tab with error details so you can diagnose and retry.
Licensing
Is there a free trial?
Yes. Every new subscription starts with a 30-day free trial — no payment required upfront. You get full access to all features during the trial.
What does the license include?
Your license is valid for the length of your subscription and includes updates and support. Licenses are per care home (seat-based).
How is it licensed?
Licensing is seat-based — each seat covers one care home. Seats and subscriptions are managed through the app.formconnector.co.uk portal, where you can add or remove seats, view billing, and manage your account.
