Introduction
Welcome to the documentation for the CareHQ WordPress Form Connector. This plugin connects your WordPress forms directly to CareHQ CRM, automatically sending enquiries from your website into the right care home location — no manual data entry needed.
Requirements
- WordPress 6.0 or higher
- PHP 8.0 or higher
- An active CareHQ account with API access
- At least one supported form plugin installed

Key Features
- Multi-form support — connect forms from any supported plugin to CareHQ
- Care home management — link CareHQ locations to your site with seat-based licensing
- Field mapping — map form fields (first name, last name, email, phone, message) to CareHQ contact fields
- Conditional routing — route submissions to different care homes based on a form field value (e.g. a "Preferred Home" dropdown)

Plugin Overview
Once installed, the plugin adds six tabs to your WordPress admin:
