The agreement between you and Formconnector when you use the CareHQ WordPress Form Connector.

1. Introduction

These Terms & Conditions ("Terms") govern your use of the CareHQ WordPress Form Connector software, website, and related services (collectively, the "Service") operated by Formconnector ("we", "us", or "our").

By accessing or using the Service you agree to be bound by these Terms. If you do not agree, please do not use the Service.

2. Third-Party Disclaimer

Formconnector is an independent company and is not affiliated with, endorsed by, or officially connected to CRMHQ Limited or their CareHQ® platform in any way.

The CareHQ WordPress Form Connector is a third-party integration tool designed to work with the CareHQ® API. CareHQ® is a registered trademark of CRMHQ Limited. All references to CareHQ® on this website are for descriptive purposes only and do not imply any partnership, sponsorship, or endorsement.

We are solely responsible for the Service. Any issues arising from the use of our plugin should be directed to us, not to CareHQ® or CRMHQ Limited.

3. Accounts & Subscriptions

To use certain features of the Service you must create an account and select a subscription plan. You agree to provide accurate, current, and complete information and to keep your credentials secure.

  • You are responsible for all activity that occurs under your account.
  • We reserve the right to suspend or terminate accounts that violate these Terms.
  • Subscription fees are billed in advance on a recurring basis according to your chosen plan.

4. Licence & Usage

Subject to these Terms and payment of applicable fees, we grant you a limited, non-exclusive, non-transferable, revocable licence to install and use the CareHQ WordPress Form Connector WordPress plugin on the number of sites permitted by your subscription plan.

You must not:

  • Reverse-engineer, decompile, or disassemble the plugin.
  • Redistribute, sublicence, or resell the plugin or any licence keys.
  • Use the Service for any unlawful purpose or in violation of any applicable laws.
  • Attempt to interfere with the security or integrity of the Service.

5. Data & Privacy

Your use of the Service is also governed by our Privacy Policy. The plugin facilitates the transfer of form submission data from your WordPress site to the CareHQ® API. You are responsible for ensuring that you have appropriate consent from individuals whose data is processed through the Service.

6. Availability & Support

We aim to keep the Service available and reliable, but we do not guarantee uninterrupted access. We may perform maintenance, updates, or modifications at any time.

7. Intellectual Property

All content, code, design, and branding associated with Formconnector and the CareHQ WordPress Form Connector are the intellectual property of Formconnector unless otherwise stated.

CareHQ® and associated branding are the intellectual property of CRMHQ Limited. Our use of the CareHQ® name is strictly for descriptive purposes to indicate compatibility.

8. Limitation of Liability

To the fullest extent permitted by law, Formconnector shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or in connection with the Service, including but not limited to:

  • Loss of data, revenue, or business opportunities.
  • Errors, interruptions, or delays in the Service.
  • Actions or omissions of third-party services, including CareHQ®.

Our total liability to you for any claim arising from these Terms shall not exceed the amount you paid to us in the twelve (12) months preceding the claim.

9. Termination

You may cancel your subscription at any time through your account dashboard. We may terminate or suspend your access if you breach these Terms.

Upon termination, your licence to use the plugin ceases immediately. We are not obligated to retain any data associated with your account after termination.

10. Changes to These Terms

We reserve the right to modify these Terms at any time. Material changes will be communicated via the Service or by email. Your continued use of the Service after changes are posted constitutes acceptance of the revised Terms.

11. Governing Law

These Terms are governed by and construed in accordance with the laws of England and Wales. Any disputes shall be subject to the exclusive jurisdiction of the courts of England and Wales.
