Form Setup

The Mappings tab is where you connect your WordPress forms to CareHQ. The plugin automatically detects all forms across your installed form plugins.

Selecting a Form

1. Go to the Mappings tab in the plugin admin.
2. You'll see a list of all forms detected across your active form plugins (Gravity Forms, Contact Form 7, WPForms, Forminator, Fluent Forms). Use the filter to narrow by plugin if needed.
3. Click on a form to open its configuration.

Configuring a Form

Each form has four sub-tabs:

Settings

• Enable/disable the CareHQ integration for this form.
• Default location — choose which care home receives submissions when no routing rules match.

Field Mappings

Map each CareHQ contact field to the corresponding form field. See the Field Mapping guide for details.

Routing

Route submissions to different care homes based on a form field value. See the Conditional Routing guide for details.

A visual node-based graph showing your submission pipeline — from trigger through field mapping, routing, locations, and the final CareHQ submission.

Testing

1. Submit a test entry through your form on the front end of your site.
2. Go to the Logs tab — a successful submission shows a “Success” status with the submitted data.
3. Check your CareHQ account to confirm the enquiry arrived at the correct location.
